Licensed Practical Nurse (LPN)The Licensed Practical Nurse (LPN) performs nursing functions within the scope of practice and provides and/or coordinates care and services to meet the residents' physical, psychosocial, and functional care needs.Job ResponsibilitiesPractices nursing care within the scope of licensure and in accordance with the state Nurse Practice Act; Keeps a current active license; Follows state and federal licensure rules and regulations for the level of care in which they practice and in accordance with EverTrue clinical and administrative policies and proceduresWorks collaboratively with the interdisciplinary team in defining individualized plans of care for each resident; Works closely with nursing leadership to ensure regulatory compliance; Promotes individual and team learning and provides teaching and role modeling of professional nursing practices to staff for quality clinical carePasses medication and/or oversees the medication ordering and administration process; Administers medications and treatments to residents; monitors administration of I.V. fluids, supplemental nourishments, and medications when orderedMay initiate IV therapy if the LPN is IV-certifiedMaintains a working knowledge and understanding of the observation/assessment and documentation process for the level of licensure in which they work, including care and service plans and the MDS as applicableUtilizes the electronic medical record to capture full clinical documentation in accordance with policy and proceduresObserves resident status as necessary to assure quality care; Coordinates with other resident services to assure continuity of care; Documents medical information and maintains detailed, timely, and accurate reportsMaintains a safe, comfortable, and functional environment by assessing potential hazards; Implements practices to maintain optimum safetyOversees and directs the certified staff and others as assigned to ensure the EverTrue standards of care and services are providedReports and supports investigative protocols as it relates to staff or resident incidents and/or accidents in accordance with state and federal regulationsQualifications, Knowledge, Skills & AbilitiesLPN licensure in state of practiceMust be CPR certifiedSpecialized training or experience in gerontological nursing preferredPhysical Requirements and Working ConditionsThe physical requirements and work environment described here are representative of those an employee encounters while performing the essential functions of this job.While performing the duties of this job, the employee must be able to lift 30lbs, and push/pull 50lbs.
