Licensed Practical Nurse (LPN) – Wound Care Job Type: Full Time | Schedule: 8:00 AM – 4:30 PM

Responsibilities Contribute to patient assessment by collecting data for RN analysis.

Perform plan of care interventions, including specimen collection and wound care; may administer medication.

Document patient care and assist RN in development of the plan of care.

Assist patients with daily living activities: personal hygiene, elimination, nutrition, and ambulation.

Monitor, document, and communicate patient status and responses to interventions with RN, physician, patient, and family.

Advocate for the physical and emotional well‑being of the patient.

Participate in performance improvement and continuing education.

Provide quality patient care and work flexible shifts/units as needed.

Maintain regular and reliable attendance.

Perform other duties as assigned.

Qualifications Associate’s Degree – Required.

Current LPN license in the state.

At least 1 year of wound care experience.

Critical‑thinking skills, decisive judgment, ability to work with minimal supervision.

Ability to work in a stressful environment and take appropriate action.

Certification: BCLS or receive within one week of hire/transfer.

Collaborate with RNs and providers, input into development and maintenance of nursing care plan, make and document nursing observations.
