We provide a wide variety of out-patient physical therapy services including sports and orthopedics, vestibular/concussion, pelvic health, pediatrics, athletic training outreach services, wellness plans, etc.

We are seeking a dedicated Physical Therapist to join our healthcare team in Eden Prairie. As a vital member of our rehabilitation services, you will provide comprehensive physical therapy care while working collaboratively with our multidisciplinary team to deliver optimal patient outcomes.

Implement evidence-based physical therapy interventions and monitor patient progress
Document patient assessments, treatment plans, and progress notes with attention to detail
Supervise and coordinate care delivered by physical therapy assistants and rehabilitation technicians
Collaborate with referring physicians and other healthcare providers
Maintain accurate and timely documentation in compliance with healthcare standards
Utilize rehabilitation equipment and techniques effectively
Participate in regular team meetings and care coordination discussions
Graduate degree from a CAPTE-accredited Physical Therapy program
Current Minnesota state physical therapy license or eligibility to obtain one
Strong knowledge of anatomy, physiology, and rehabilitation principles
Strong documentation and electronic health record skills
Ability to work independently and as part of a healthcare team
