We are hiring an Admission Registered Nurse (RN) for a Home Health position providing skilled nursing care to patients in their homes in Jackson and Clay Counties. This position is eligible for a $8,000 first year success bonus to reward the accomplishment of key achievements during your first year!

Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders
Educate and guide patients and families through the admission process, setting expectations for care, reviewing diagnoses and treatment plans, and ensuring they feel informed and supported from day one
Apply strong clinical judgment and critical thinking to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation
Work independently, but never alone
Travel to patient homes within assigned territory

Admission RN Qualifications

Active Registered Nurse, RN license in the state you work
One year of RN experience and the clinical expertise to deliver quality patient care
One year of home health experience as an RN, preferably conducting OASIS assessments.
