Job Title: Occupational Therapist (Skilled Nursing & Rehab)

Position Summary
We are seeking dedicated and compassionate Occupational Therapists (OTs) to deliver therapy services in skilled nursing and rehab facilities nationwide. This role serves a mixed caseload of short-term rehab and long-term care residents, focusing on restoring and enhancing patients' independence in activities of daily living (ADLs), functional mobility, and overall quality of life.
Key Responsibilities
Perform evaluations to assess patients' functional status, needs, and goals.
Develop and implement individualized treatment plans for patients with physical, cognitive, or psychosocial impairments.
Provide direct therapy for both short-term post-acute rehab patients and long-term residents.
Focus on ADL and instrumental ADL retraining, upper extremity rehabilitation, adaptive equipment training, and energy conservation techniques.
Collaborate with physical therapists, speech-language pathologists, nurses, and other interdisciplinary team members to ensure coordinated care.
Educate patients, families, and caregivers on home programs, adaptive techniques, and equipment use to maximize independence.
Complete timely documentation of evaluations, progress notes, treatment sessions, and discharge summaries in accordance with state/federal regulations and facility policies.
Participate in care planning meetings and contribute to discharge recommendations.
Maintain compliance with Medicare, Medicaid, and other payer guidelines for therapy services.
Promote a safe, respectful, and therapeutic environment for residents.
Required Qualifications
Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited OT program.
Current, active Occupational Therapist license in state of practice (or eligibility to obtain).
NBCOT certification in good standing.
CPR/BLS certification.
Previous experience in Skilled Nursing Facilities, inpatient rehab, or mixed short/long-term care settings preferred.
Strong communication, collaboration, and organizational skills.
Familiarity with EMR/electronic charting systems preferred.
New graduates with strong clinical rotations in SNF/rehab considered.
