Advanced ICU Nurse - Full time

Do you have a passion for helping the Geriatric population? Tapestry Health is seeking an on-site Nurse Practitioner or Physician Assistant to join our team full-time providing medical care to patients in Skilled Nursing Facilities located in Phenix City, Alabama.

The Nurse Practitioner will diagnose and treat health conditions and work collaboratively with the facility medical staff.

Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.

Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident.

Prescribe medications and treatments within the scope of practice and in accordance with state regulations and facility policies.

Participate in care initiation for our chronic care management services.

Provide education and counseling to residents and their families regarding health maintenance, disease prevention, and self-care strategies.

Collaborate with physicians, nurses, therapists, social workers, and other healthcare professionals to ensure coordinated and holistic care delivery.

Participate in quality improvement initiatives and interdisciplinary meetings to enhance the overall quality of care and resident outcomes.

Maintain accurate and up-to-date documentation of all assessments, interventions, and communications in the electronic medical record system within 72 hours of seeing patient.

Current licensure as a Nurse Practitioner in [INSERT STATE]

Experience with providing care in ICU, ER, Urgent Care, Primary Care, and Surgical will be considered.

Proficiency in using electronic medical record systems and other healthcare technology tools.

Ability to travel occasionally to facilities within your region as needed to complete initiating patient visits.
