Occupational Therapist (OT) - PACE Program. Graduate of an AOTA-accredited Occupational Therapy program
Current BLS and First Aid certification
1+ year working with older adult populations
Experience with assistive technology and adaptive equipment
Fluency in Cantonese or other languages a plus

Job Overview The Occupational Therapist plays a vital role in supporting older adults enrolled in the PACE (Program of All-Inclusive Care for the Elderly) model. Working independently under the supervision of the PACE Center Manager, the OT collaborates closely with the interdisciplinary team to deliver holistic, participant-centered care.
This role blends clinic-based therapy, home safety assessments, functional rehabilitation, and care planning to support independence, safety, and quality of life. The OT partners closely with the Physical Therapist and other clinical team members to deliver a comprehensive, individualized rehabilitation program.
Provide therapy services in the PACE center, participant homes, and via telehealth
Perform home safety assessments and coordinate adaptive equipment installation
Oversee therapeutic activities in the rehabilitation gym
Educate participants and families on safety, function, and rehabilitation strategies
Supervise aides and staff supporting therapy programs
Document evaluations and interventions in Epic
Participate in quality improvement initiatives and team meetings
