TRIAGE NURSE Full-Time
Live Well Homecare
Role Registered Nurse in a Medical Group setting, providing on-site patient care.
Deliver nursing services under the supervision of Practice Managers and Physicians.
Perform phone triage, interpret test results, manage medication refills, prior authorizations, and patient education.
Review and respond to patient messages via electronic health records.
Maintain accurate documentation in Epic EMR.
Qualifications and Skills Current Illinois RN license.
Graduate of an NLN-accredited nursing program; BSN preferred.
BLS/CPR certification required.
At least 1 year of nursing experience in an office or hospital setting.
Key Duties and Requirements Full-time position with on-site work, Monday to Friday, with rotating weekends.
Provide compassionate, professional nursing care with attention to patient communication and record-keeping.
Ability to travel as needed within the department.
