WEEK END REGISTERED NURSE (RN) Job Title: Registered Nurse

Reports To: This position reports to the Director of Nursing (DON) or designated clinical leadership. The RN functions as part of the interdisciplinary care team and provides supervision and direction to licensed and unlicensed clinical staff as applicable.

The Registered Nurse (RN) is responsible for providing and coordinating skilled nursing care for patients in the home setting. The RN completes comprehensive assessments, develops and implements patient care plans, and ensures care is delivered in accordance with physician orders, Medicare Conditions of Participation, state regulations, and agency policies.

The RN serves as a key member of the interdisciplinary team and ensures safe, effective, and compliant care delivery.

Patient Assessment & Care Planning

The RN participates in start of care (SOC), recertification, and reassessment visits as required.

Develop, implement, and update individualized patient care plans

Evaluate patient response to care and adjust interventions as needed

Ensures physician orders are appropriate and consistent with the patient’s clinical condition and care plan, and communicates discrepancies as needed.

Skilled Nursing Care & Coordination

Provide skilled nursing services in accordance with physician orders and care plans

Coordinate care with physicians, therapists, and interdisciplinary team members

Ensure continuity of care across all services provided

Monitoring & Clinical Decision Support

Communicate significant changes promptly to physician and care team

Ensure documentation supports medical necessity and compliance requirements

Maintain adherence to Medicare Conditions of Participation and agency policies

Completes OASIS assessments accurately and within required timeframes in accordance with Medicare regulations and agency policy.

Supervision & Team Collaboration

Provide direction and supervision to LPNs and CNAs as applicable

Participate in interdisciplinary team meetings and care coordination

Support clinical education and guidance for field staff

Graduate of an accredited School of Nursing (ASN or BSN required). Current Florida Registered Nurse (RN) license in good standing.

Minimum of 1 year nursing experience required

Home health, hospice, or acute care experience preferred

Valid driver’s license, reliable transportation, and auto insurance

Ability to work independently in a home health environment

Commitment to compliance, quality care, and patient safety

Professionalism and reliability in field-based care

Ability to travel to patient homes within assigned territory

Ability to communicate effectively in person, by phone, and electronically

Work is performed in patient homes with varying environmental conditions, including exposure to pets, smoke, infectious diseases, and weather-related travel conditions.
