We are searching for a qualified ACT Team Lead for our Jackson, MI, office.

POSITION SUMMARY The Mental Health Advocate provides person-centered, community-based behavioral health services to individuals receiving Assertive Community Treatment (ACT) services. This position promotes recovery, independence, and community integration through assessment, advocacy, care coordination, crisis intervention, skills development, referral and linkage, and ongoing support. Working as part of a multidisciplinary ACT team, the Mental Health Advocate partners with consumers, their natural supports, and community providers to develop and implement individualized treatment plans that promote improved health outcomes and quality of life.

Success in this position requires excellent interpersonal communication, sound clinical judgment, effective time management, accurate documentation, collaboration within a multidisciplinary team, and a commitment to person-centered, recovery-oriented care.

Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Criminal Justice, Nursing, Education, or related field

Qualified Mental Health Professional (QMHP) or ability to meet QMHP qualifications under Michigan Medicaid requirements

Ability to safely transport consumers in personal or agency vehicles

Ability to document services accurately in an Electronic Medical Record

Reside within a 45-minute radius of the local hospital to ensure readiness for possible on-call duties

Experience in community mental health

Knowledge of person-centered planning

Trauma-Informed Care

Experience utilizing an electronic medical record system

Participate in interdisciplinary treatment planning

Deliver community-based behavioral health services

Care Coordination Coordinate with PCPs, hospitals, community agencies, and other related service providers

Collaborate discharges and continuity of care

Documentation & Compliance Complete timely, accurate, and clinically appropriate documentation in accordance with Medicaid, CARF, agency, and regulatory requirements

Consumer Advocacy Assist consumers in obtaining, maintaining, and maximizing benefits, entitlements, housing, employment, transportation, and other community resources

Promote consumer recovery, independence, self-advocacy, and successful community integration

Follow all agency policies, procedures, and standards of conduct

Demonstrate sound judgment and make decisions that align with the agency's mission, values, policies, and ethical standards

Exhibit professionalism, accountability, and reliability in attendance, communication, and work performance

Treat consumers, families, visitors, coworkers, and community partners with dignity, respect, and compassion

Participate in required training and maintain all required licenses, certifications, and professional competencies

Support quality improvement initiatives by identifying opportunities for improvement, participating in organizational improvement efforts, and contributing to a culture of continuous learning

Comply with Corporate Compliance policies, Recipient Rights requirements, CARF standards, Medicaid requirements, the Michigan Mental Health Code, and other applicable regulatory requirements.

Safely operate agency or personal vehicles when required and maintain a valid driver's license and automobile insurance in accordance with agency policy and Michigan law
