Urgent December Coverage Needed
Anthony Hospital is looking for urgent December coverage with their Gastroenterology team. The site requires ERCP and EUS to be considered.
Orientation $262.50 - $275 - half the hourly/callback rate, kicks in after 8 hours. "Per hour of orientation or training beyond 8 hours"

• 24 Hour Holiday Call Rate - $7800 - 8400 - "Per 24 hours to include 8 hours gratis"
• Call Back $525 - $550 "Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms."
• Holiday Call Back $787.50 - 825 "Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms."

Average callback hours: Weekday: 11.63 hours; Weekend: 10.38 hours
All shifts end the following morning at 7a.
Level 1 Trauma
~ Scoping, ERCP, EUS, Endoscopy, GI Bleed, Evaluation of GI problems.

~ EMR: EPIC

~ EMR Required Training:
Credentialing Preapp and full app must be completed and submitted to move provider into "applicant in training" to generate AD account and begin EMR account set up. Once AD account creation begins it can take up to 5 days to create account.
Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
EMR Virtual (PPL) training Only able to attend if Modules are completed before training. Virtual training must be completed at least one week prior to the providers first day.
First Day Details - Site requires completion of EMR Modules and training, before first day details can be released.

Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.
Credentialing Information:
Timeframe: 60-90 day standard, Emergency privs 1-2 weeks with clean file.
Credentialing with other facilities? Preapp and full app must be submitted within right away for urgent dates needed to allow for AD account creation with EMR.

• Adult, Geriatric, Adolescent
• 15 years old and 50 kilograms for a pediatric trauma. Otherwise, Pediatric patients are not admitted to the facility. GI does not admit they are just a consult service. 24/7 intensivists & hospitalists at site.
• 24/7 Trauma Services at site.

Board Certified Gastroenterology - [REQUIRED]
Active DEA - [REQUIRED] Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)
Candidates must detail the # of shifts per month ongoing they can offer to the site - [REQUIRED]
