Hospital Liaison Nurse LPN

Powers Health

The Intake Nurse at Powers Health is an LPN/RN-qualified professional responsible for receiving, processing and managing agency referrals and assisting with hospital to home discharge. You will liaise with social service and case management teams across hospitals and physician offices.
Candidates should graduate from an accredited RN/LPN program, have a minimum of 2 years of nursing experience (1 year in home health preferred), and hold Indiana licensure with CPR certification.
