Job Description

As an LPN with Southern Health Partners, you will:

Deliver direct patient care and medication administration

Conduct nursing assessments and intake screenings

Assist with emergency response and triage situations

Monitor chronic care patients and treatment plans

Maintain accurate documentation and medical records

Collaborate closely with RNs, providers, and correctional staff

Help create a safe, professional, and compassionate care environment
