Health Care Management Nurse

This RN position provides skilled nursing care to Interim HealthCare patients residing in assisted living facilities throughout the Upstate SC area, with occasional home visits as needed. The RN will collaborate with assisted living facility staff and healthcare providers through ongoing weekly interdisciplinary communication to ensure coordinated, quality patient care.

As a Registered Nurse (RN), you will:

Provide skilled, patient‑centered care in the home while partnering with physicians and the interdisciplinary team to support safe recovery and long‑term health

Develop, implement, and adjust individualized plans of care

Monitor patient condition and communicate changes promptly to physicians to prevent complications

Coordinate care with therapists, aides, and other providers to ensure seamless, compliant care

Prepare patients and families for safe, successful discharge and document care accurately in the EMR

Graduate of an accredited Registered Nurse (RN) program

Active South Carolina RN license or Compact License

Minimum one year of nursing experience

Experience in Home Health, Medical Surgical, Critical Care, or Case Management preferred
