Medical, Vision, and Dental plans through BCBS28 days of Paid Time OffExcellent mileage reimbursement rate403b Retirement plan with matchingFocused programs honoring Veteran patientsAssistance with achieving Certified Hospice & Palliative Nurse (CHPN)Best Orientation and Onboarding program you've experiencedSeasoned Hospice leaders guiding your career growthEssential FunctionsDelivery of Patient Care Assumes responsibility for the coordination and delivery of patient/caregiver care.Conducts nursing assessments to assist in the overall management of patient/caregiver needs.When appropriate, develops, implements, evaluates, and updates the plan of care to ensure that care is uninterrupted and that goals are achieved.When providing hospice care, appropriately utilizes VIA Health Partners patient related protocols, standing orders and guidelines.Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care.Applies principles of infection control. Interdisciplinary Group Attends, participates, and may facilitate interdisciplinary group conferences [IDG] and special care conferences.Documentation Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.Professional and Personal Accountability Demonstrates competency in knowledge and skills.Ensures that VIA Health Partners professional reputation is maintained and projected.Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Maintain productive and professional relations with external agencies and healthcare providers.Able to plan workday while remaining able to respond appropriately to crisis.Ensures all areas of facility are clean and appropriately stocked with supplies. Ensures proper chemicals are used for cleaning different surfaces. Ensures protective equipment is used when working with chemicals, i.e., QualificationsMinimum QualificationsA Diploma or associate degree in nursing is required; Bachelor's Degree preferred.A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions. Experience in direct care of patients with life limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.Computer proficiency is required
