Full Time REMOTE Dietitian

Clinical Registered DietitianUpperline Health is seeking a Clinical Registered Dietitian to provide nutritional services to our high-risk, complex care patient population, which includes counseling and nutritional assessments, as a member of the interdisciplinary team to improve results and maximize outcomes.Ideal candidates will have experience working with an older adult population (65+). This role involves addressing a diverse range of topics, including weight loss, CKD, diabetes, heart health, gout, and kidney stones. Most importantly, candidates must demonstrate compassion for patient needs, have great attention to detail and thrive in a collaborative environment.*Must have active FL licensure.What You'll DoEducate the interdisciplinary team on dietary and nutrition guidelines to support health maintenance in the older adult population with chronic illnessesObtain and evaluate nutrition histories of patients regarding food habits; assess patients' knowledge of diet and health maintenance and provide appropriate detailed education to patients and their families regarding nutritional status, adequacy measures, fluid status, and all nutrition-related lab parameters pertaining to their disease processComplete written nutritional assessments as well as comprehensive patient assessments for patients' written care plans in accordance with established Federal and State licensure guidelines; participate in care plan meetingsMonitor patients' adherence and response to nutrition therapy, and adjust therapy as needed to achieve and sustain an effective nutritional status; document patients' response to therapy in progress notesComplete other related duties as deemed necessary by Upperline HealthQualifications of the Clinical DietitianRegistered Dietitian in FL required; GA licensure a plus.Licensed Dietitian Member of the American Dietetic Association preferredExperience in adult clinical area preferred, or at least 2 years work experience in adult clinical dieteticsExperience working with patients with complex, unmanaged Diabetes, Hypertension, and other chronic illnessCandidates should be excited and energized by the prospect of working collaboratively as part of an innovative teamPrior healthcare experience serving chronically ill patients living with limited resources and/or mental illness preferredVirtual (phone or video chat) case management or care coordination experience preferredStrong customer-service orientationPatient, compassionate and professional demeanor requiredExceptional organizational skills and ability to multi-taskSelf-starter with an entrepreneurial spirit and demonstrated problem-solving skillsStrong writing, Word, Excel, and database management skillsExtremely strong work ethicExperience working in a fast-growing, rapidly changing environmentBenefitsComprehensive benefit options include medical, dental and vision, 401K and PTO.CompensationCompensation is commensurate to compensation for similar positions in the region and based on prior training and experienceJob Type: Full-time (fully remote)Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
