Provides holistic, quality and cost-effective nursing care for patients/families and/or others in the pre-hospitalization population within a unit, service or other primary area of practice; focuses on discharge planning which includes identification of risk factors and education. Applies the nursing process within the context of the organization‘s Framework for Professional Practice. Reports to Director, Care Continuum.

In this position the nurse will complete the Pre-Screening Call and complete preoperative chart reviews in Epic. During this call the nurse will provide holistic nursing care for patients/families/caregivers prior to hospitalization. The nurse will be responsible for accurately obtaining, assessing and identifying potential risks and barriers to expected outcomes. Patient care will be provided in partnership with a provider; including phlebotomy, EKG, and other preoperative testing as ordered.

• 3 years’ acute care experience required
