Job Description Full‑time Hospice RN on‑call

Friday 5:00 pm–11:00 pm; Saturday/Sunday 8:00 am–11:00 pm

Territory: Respond to on‑call needs within an hour

Perform physical and psychosocial assessment on all hospice patients

Assess and re‑evaluate pain; apply appropriate pain‑management techniques and educate patient and family

Respond to the physical, mental, social, and emotional needs of terminally ill patients and their families

Assess patient’s condition, initiate plan of care, re‑evaluate and update as needed

Obtain necessary physician orders and collaborate with physician as patient’s needs dictate

Initiate case conferences to discuss multidisciplinary team responsibilities, pain management, and bereavement counseling

Requirements One year hospice experience required

One year RN experience required

Licensed Registered Nurse in the State of Indiana

Current driver’s license

Reliable transportation and insurance
