Physiotherapist I - Home Care Rehab

This is a contract, on-site Physical Therapist role based in the Austin, Texas Metropolitan Area. The Physical Therapist will provide one-on-one in-home evaluations and treatments, developing and implementing individualized care plans that promote functional mobility, safety, and independence. Responsibilities include performing comprehensive physical therapy assessments, setting measurable goals, and delivering evidence-based therapeutic interventions such as exercises, gait training, balance work, and pain management strategies. The role involves educating patients, caregivers, and families on home exercise programs, adaptive equipment use, and fall prevention, while maintaining clear, timely, and accurate clinical documentation. The Physical Therapist will collaborate with nurses, physicians, and other members of the care team to adjust treatment plans as needed and support positive patient outcomes in the home setting.

Strong foundation in Medicine and clinical principles relevant to home health and rehabilitative care.

Demonstrated expertise in Physical Therapy, including evaluation, treatment planning, and evidence-based interventions.

Experience in Rehabilitation, with a focus on restoring function, mobility, and quality of life for diverse patient populations.

Proven commitment to high-quality Patient Care, emphasizing safety, dignity, cultural sensitivity, and clear communication.

Knowledge of Home Care practices, including in-home safety considerations, caregiver training, and community resource coordination.

Current state licensure as a Physical Therapist (PT) in Texas and graduation from an accredited physical therapy program.

Previous experience in home health, geriatric care, or post-acute rehabilitation preferred.

Ability to work independently in the field, manage a mobile schedule, and maintain accurate electronic documentation.

Valid driver’s license, reliable transportation, and ability to travel within the Austin, Texas Metropolitan Area.
