Haven Hospice is seeking a passionate, dedicated Social Worker to join our team inDeland, FL. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect. Develop deep meaningful relationships during a personal and significant life stage
More quality time with patients, allowing for more thoughtful unrushed care
assist with alternative care plans when needed
Serve as liaison with community resources and agencies
Collaborate with team members to support holistic patient care
Support funeral, financial, and legal planning with families

*A graduate of a master's program in social work (MSW) accredited by the Council on Social Work Education OR A graduate of a bachelor's program in social work accredited by the Council on Social Work Education. Minimum of one (1) years' experience in health care, hospice experience preferred. Experience in a hospice care preferred.
Current licensure according to Medicare guidelines to practice in state of operation.
