We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in our adult residential facilities in New Castle, IN.

Only on call for triage of emergencies 1 week per month

Mileage & Cell Phone Reimbursement

Flexible Hours -5 day work week (40 hours, Monday thru Friday) NO Weekends Typically 8am-5pm

Case management role with semi-annual physical assessments.

Paid training

Ensure that each individual receives quality, individualized health care management, resulting in their optimal health

Conducting routine site visits, checking vitals, doing assessments, counseling of medication administration, and documenting progress

Act as a health/medical resource for staff, training staff in the medical arena

Ensure that the company is in compliance with state requirements and organizational policies regarding medical care and records

Full-time Monday-Friday with occasional travel required; Participate in an on-call rotation for triage of medical concerns for 1 week per month

Current Licensed Practical Nurse (LPN) license in the State of Indiana required

• 1-2 years experience working with individuals with disabilities or other long-term care is preferred
• Organizational and computer skills
