Weekends 8:00AM to 4:00PM or 12:00AM to 8:00AM

Provide direct patient care in the home
Monitor patient condition and report changes
Administer medications and treatments as ordered
Support individualized care plans
Document care and communicate with the healthcare team
Requirements
Required:

Active LVN license
Current BLS certification
Preferred:

~1+ year of nursing experience
