Job Summary: Provides direct patient care under the supervision of a Registered Nurse or other healthcare professional, following the established plan of care and treatment goals.
Status: Full-Time
Shift: Monday - Friday
Responsibilities:

Observes and promptly reports changes in patient condition or status to the RN Case Manager, Clinical Supervisor, or other appropriate healthcare team members.
Accurately documents patient observations, measurements, findings, and care outcomes in accordance with established documentation standards.
Monitors and records vital signs, including blood pressure, pulse, temperature, respiration, weight, and pain levels.
Implements individualized care plans and documents patient progress and updates in a timely manner.
Assists patients with activities of daily living, including bathing, personal hygiene, mobility, transfers, ambulation, and prescribed exercise programs.
Performs supportive household tasks that promote patient independence, enhance self-care, and help prevent or delay the need for higher levels of care.
Supports the onboarding and training of new staff by providing guidance on patient care practices, documentation requirements, and effective time management.
Education/Experience/Skills/Qualifications:

• Current Home Health Aide certificate from the State of California
• Current CPR certification
• 1 year Home Health Aide experience preferred
• Bilingual in Spanish helpful

The compensation range for this role is determined by factors such as location and required skills. The salary range provided reflects the lowest to highest amount we reasonably anticipate paying for this position at the time of posting. Compensation is influenced by labor market conditions and various factors, including but not limited to: prevailing minimum wage in the area, relevant education, experience, skills, geographic location, performance, travel requirements, sales or revenue targets, and business needs. It is not typical for a new hire to be offered the highest end of the salary range. Pay decisions are based on the unique circumstances of each candidate and position.
