Responsibilities:

Identifies cognitive impairments, mood problems and other psycho-social needs in residents.
Maintain accurate, thorough, and timely documentation.
Review advanced directives and provide education to residents and family members as needed.
Help provide a smooth admission and discharge process for residents and family members.
Other duties as assigned by administrator.
Qualifications:

Ability to work independently or part of a group.
Computer knowledge.
Ability to build and maintain rapport with residents and peers.
Prior long term care/skilled nursing experience in social services or required
LSW certificate required
