MUSC Health at Home by BAYADA
Immediate opening for a Per Diem Physical Therapist, Home Health to provide home health visits for our MUSC Health at Home by BAYADA Office in Summerville, SC and surrounding areas.

As a home health care PT, you will be an integral member of a multi‑disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to remain at home.

Responsibilities

Make home visits to clients in designated geographic territories.

Perform diagnostic tests and measurements such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.

Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence‑based dressings and topical agents.

Develop and implement individualized care plans including manual therapeutic exercises, gait training, balance, and other interventions.

Continuously assess and revise the PT care plan, and collaborate with nursing in the multidisciplinary care plan as appropriate.

Educate and instruct clients, family members, or other client representatives on rehabilitative care and activities needed to promote client health, safety, and independent living.

Accurately document observations, interventions, and evaluations using a state‑of‑the‑art touch‑pad tablet.

Qualifications

Current SC Physical Therapist license.

Minimum one year of recent work experience as a Physical Therapist.

Graduated from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, evidenced by transcript or diploma.

Ability to work independently and manage time effectively.

Strong interpersonal, organizational, and problem‑solving skills.

Solid computer skills; prior EMR experience preferred.
