Centra's PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older in our community. The mission of PACE is to deliver person-centered care in the setting that's most appropriate for each participant—whether that's in their own home, with family, in long-term care, or assisted living. PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs. Everything is coordinated under one roof with a dedicated interdisciplinary team of providers, nurses, dietitians, physical and occupational therapists, social workers, activities coordinators, and transport staff. Centra's PACE program is seeking a dedicated Advanced Practice Provider to join our team and help shape the future of our care model. Graduate of an accredited Nursing Program, and Masters of Science Degree in Nursing (MSN)Required Experience: 2 years of Nurse Practitioner experienceRequired Certifications and Licensures: Current licensure by the Virginia State Board of Nursing as a Nurse Practitioner with prescriptive authority. Hold a current, active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse. Nationally recognized certification in field of specialization
