The Registered Nurse (RN) – Night Shift will join a highly skilled, compassionate interdisciplinary clinical team dedicated to providing patient- and family-centered hospice care. YoloCares' clinical team includes registered nurses, hospice aides, social workers, spiritual care counselors, nurse practitioners, and physicians who support patients and families through every stage of the hospice journey. The team is grounded in clinical excellence, mutual respect, and thoughtful coordination, with a shared commitment to dignity, comfort, and individualized care.
The RN – Night Shift reports to the Clinical Manager and is responsible for providing direct patient care under the supervision of the Director or designee. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
Triage phone calls from patients/families.
Work cooperatively with patients, families, nursing staff, physicians, consultant personnel, and ancillary service providers.
Report to DPCS/Clinical Manager and day shift nurses with patient updates from night shift with daily reports and EMR charting.
Work cooperatively with patients, families, nursing staff, physicians, consultant personnel, and ancillary service providers.
Identify personal education needs and seek formal programs or participate in educational activities or seek other avenues to meet these needs.
Participate in the evaluation(s) of own performance by identifying strengths and weaknesses and by developing a plan to improve areas of weakness.
California RN license required, and CPR certified.
Minimum of one year hospice experience desired; two years SNF/home care experience preferred.
Must be knowledgeable, at minimum, about Hospice Medicare Conditions of Participation and California Hospice Licensing Standards.
Ability to work effectively with community physicians and other health care providers (all disciplines) toward specified goals is required.
Knowledge and experience using electronic medical software systems (EMR), Allscripts a plus.
Must be computer literate in Microsoft Word. PowerPoint software is preferred.
Second language is a plus.
Ability to safely lift medical equipment (e.g. a wheelchair) weighing up to 35 pounds and to assist family/caregivers with transferring and ambulating patients as safety requires; Must have visual acuity and hearing to perform required nursing tasks and duties.
Ability and willingness to flex or add work hours to meet scheduled shifts and holidays is required.
Ability and willingness to maintain and drive a personal vehicle. Ability to work 7 days on and 7 days off shift.
