Job Summary

The position is under the general direction of the Facility Manager. The main focus is providing superior outpatient rehabilitation services through a collaborative care model with our physicians in order to optimize performance and enhance the quality of function for patients.

Responsibilities

Exceptional patient care

Interpret physician orders, perform initial evaluations, and establish treatment plans and functional goals

Upgrade and amend treatment goals as necessary during the episode of care

Write documentation on patient status to physicians per compliance guidelines of IBJI rehab

Determine appropriate discharge timeline and follow‑up

Supervise rehabilitation technicians with patient care and administrative staff as needed

Comply with daily paperwork; PT/PTAs/ATCs must document all treatment in patient charts promptly (initial evaluations, daily notes, progress notes, discharges, HEP)

Stay current with billing policies and demonstrate ethical billing practices

Maintain active state license by earning appropriate CEUs through outside courses and IBJI‑sponsored educational opportunities

Develop positive relationships with the physician teams at IBJI

Take an active role in the patient medical team: office visits with physicians, contact with case managers, etc.

Participate in programs sponsored by IBJI: mentoring, clinical educator, special interest group, community outreach

Requirements

Graduate from an accredited physical therapy program

Licensed or eligible to sit for boards in Illinois

Ability to work in a fast‑paced environment

Professional and effective writing and verbal communication skills

Ability to lift greater than 25 pounds

Demonstrate understanding of proper body mechanics for lifting, pulling and pushing

Ability to stand and sit for sequences of time greater than 30 minutes

Ability to work on a computer for lengthy times during the day

Salary

$32,800 - $44,000 depending on experience.
