Home-Based Hospital Care Registered Nurse

Humana Inc

Humana Inc. seeks an Onsite Care Coordination Nurse in Hazel Crest, IL to assess newly admitted members and guide them through discharge planning. The role blends in-person facility work with remote documentation, fostering a seamless care transition from hospital to home.
Qualified candidates hold an IL RN license, 3+ years in acute care, and experience with discharge planning or care coordination. A BSN is preferred, with bilingual English/Spanish a plus.
