Weekend CNA - Hospice Homecare

Personal Care Assistant
Provide personal care under the supervision of a nurse according to the patient's plan of care. Provide personal care and assist with ADL's according to the Plan of Care.
Under the direction of the nurse, perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate.
Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit.
Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures.
Attend and actively contribute during patient care conference.
Participate in the on-call rotation as scheduled (Home Care only).
Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.)
Demonstrate positive customer-service responsiveness according to agency standards.
Flexible in meeting patient/family needs that require variable shifts and weekends.
Participate in chart review quality improvement activities and other meetings as assigned.
Must be state tested nursing assistant in good standing and have 12 months of experience as a nursing assistant.

Must be a Florida Certified Nursing Aide-CNA.
