Registered Dietitian and Registered Nurse

Job Summary and Responsibilities
As a Clinical Dietitian, you will provide expert nutrition care, developing and implementing individualized plans to improve patient health outcomes. You will clearly communicate nutritional data across all systems and participate in health team meetings as the resident nutrition expert. To be successful, you will integrate robust clinical expertise with empathetic, evidence-based care, consistently adhering to professional standards and fostering effective communication with patients and healthcare teams.

Responsibilities

Perform nutritional assessment using age and gender appropriate standards, cultural, religious and ethnic backgrounds; and utilizing validated assessment tools, resulting in nutrition-related diagnosis when indicated. Conduct routine screening to identify patients at nutritional risk. Collaborate within an interdisciplinary environment to facilitate care and identify patients at nutrition risk or needing higher levels of care.

Implement targeted and specific nutrition interventions. Provide customized nutrition education to patients and/or families and caregivers. Work within an interdisciplinary environment to contribute toward overall patient care. Closely monitor, evaluate, and rescreen patient nutrition status as appropriate. Update plan of care as necessary and follow up with patients as needed.

Develop an intervention plan including appropriate feeding route, mode, and nutrition prescription unique to the patient's specific needs, in accordance with physicians' orders. Monitor and evaluate patient response to interventions by meeting specific measurable goals documented in the nutrition care plan in a timely and thorough manner in the medical record.

Collaborate with the Food and Nutrition Services team to enhance patient experience. Participate in test trays, menu reviews, periodic auditing of trayline for accuracy of diet orders, validate menu compliance with the facility Diet Manual Handbook and Nutrition Care Manual (NCM), support with patient interactive positions (dietitians, diet clerks, interns, etc.), maintain communication with patients, staff, and guests and provide excellent customer service. Participate in peer reviews and chart audits, and collaborate in Diet Manual Handbook training for all clinical staff (new registered dietitians, nurses, therapists, physicians, etc.).

Requirements

Bachelor’s degree or completion of accredited dietetics program (ACEND). Upon hire.

Registered Dietitian (R.D.), within 90 days of hire.

Registered Dietitian (R.D.)‑ADA, within 90 days.

Registered Dietitian (R.D.)‑AND, within 90 days.

Registered Dietitian (R.D.)‑CDR, within 90 days.

Registered Dietitian (R.D.)-TX, within 90 days.

Pay Range
$29.44 - $43.79 per hour
