Located in the vibrant city of Sacramento, CA, the full-time Physical Therapist role is a rewarding opportunity for a dedicated professional. The successful candidate will provide comprehensive outpatient physical therapy services, focusing on creating and implementing individualized treatment plans.

Attention to documentation is crucial, as you'll maintain detailed records of patient goals, progress, and medical necessity.

You'll oversee assistants and aides, ensuring high-quality care in compliance with state regulations and company policies. Past experience in outpatient physical therapy is preferred, enhancing your ability to manage a busy caseload with a focus on orthopedic and musculoskeletal conditions.

Collaborative in nature, you'll work closely with colleagues, including PTAs, aides, and healthcare providers, to maintain open communication in pursuit of superior patient care.

The Physical Therapist (PT) will provide high-quality physical therapy services for patients, caregivers, and facilities while adhering to state practice laws and company policies. The role involves patient evaluation, treatment planning, supervision of assistants and aides, and collaboration with healthcare providers.

Evaluate, treat, and direct skilled treatment for patients requiring physical therapy

Maintain ongoing documentation of goals, functional progress, and medical necessity

Complete required documentation (pre-certifications, physician reports, daily notes, etc.) Maintain respectful and open communication with co-workers, physicians, patients, and families

Stay updated on the latest physical therapy practices and regulations

Understand and adhere to the state Practice Act and supervisory requirements for PTs

Follow VibrantCare’s Clinical Operations Policies & Procedures Manual and complete annual reviews

Complete all required Medicare compliance training and OSHA safety training annually

Complete required continuing education courses to maintain licensure

Participate in facility meetings and professional development programs as directed by the Center Manager

Contribute to center operations and workflow improvements

Support the overall goals of the company by engaging in facility marketing efforts

Graduation from a CAPTE-accredited Physical Therapy program

State licensure/registration where applicable

Experience in outpatient physical therapy preferred

Proficiency in clinical documentation systems and Microsoft Office (Word, Excel)

Frequent walking, bending, standing, and reaching throughout the workday

Strong visual and auditory acuity for patient evaluations and documentation

Ability to push wheeled equipment and handle therapeutic devices efficiently

The ideal candidate is a licensed Physical Therapist (PT) with experience in outpatient rehabilitation or a strong interest in treating orthopedic and musculoskeletal conditions.

This individual thrives in a collaborative and supportive team environment, working closely with fellow therapists, physicians, and healthcare professionals to ensure high-quality care.
