Cruz Home Health Care LLC is seeking a compassionate, skilled Registered Nurse to join our dedicated home health team in Ontario, CA. Conduct comprehensive patient assessments and develop individualized care plans for home health patients

Administer medications, wound care, IV therapy, and other skilled nursing treatments in the home setting

Monitor and evaluate patient health status, documenting changes and reporting to physicians as needed

Coordinate care with interdisciplinary team members including physical therapists, social workers, and physicians

Ensure patient care aligns with OASIS assessments and home health plan of care requirements

Active and unrestricted Registered Nurse (RN) license in the state of California

Minimum 1 year of clinical nursing experience; home health experience preferred

Knowledge of OASIS documentation, Medicare/Medicaid home health regulations a plus

Valid California driver's license, reliable transportation, and current auto insurance

Willing to travel to Los Angeles, San Bernardino, and Riverside Counties
