As a Pharmacist, you'll be involved with multiple facets of the home infusion process as you interact with insurance companies, staff members, distributors, Physicians/Nurses, and patients to ensure accurate and safe quality service. You will use your experience as part of the Pharmacy Team to resolve problems when unforeseen roadblocks arise. Perform specific distributive and clinical pharmacy activities and related services as they pertain to providing safe and effective home infusion therapies.

Adheres to organization policies and procedures.

Coordinates and facilitates new referral/admissions through collaboration with nursing, customer service, and distribution.

Receives, reviews, and verifies all new patient orders and documents are the same on the order form.

Assists the physician and other healthcare professionals involved in planning and implementing patient care in the home to ensure safe and effective drug therapy.

Monitors the outcomes from drug therapy by collecting, evaluating, and interpreting the results of laboratory data, clinical data, and other pertinent patient information.

Supervises, coordinates and/or participates in the preparation of sterile admixtures, dispenses all pharmaceutical and related supplies and equipment in accordance with federal, state, and local laws and standards of practice which govern the profession of pharmacy.

Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.

Maintains pharmacy records in accordance with applicable laws and regulations.

Participates in on-call rotation if applicable.

Participates in orientation/training programs for new personnel and current staff.

Performs other tasks or special projects as requested by management.

A Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy (PharmD) from an accredited (Council on Pharmaceutical Education) college of Pharmacy.

Currently Licensed as a registered pharmacist in the state of employment.

Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses, and other healthcare professionals.

Must be flexible with work schedule.

Dependable in attendance and job performance.
