Purpose

This role works across all programs in the Community Living Alliance Care Solution's team to provide direct oversight and supervision to Personal Care Workers. It ensures clients receive quality service and the opportunity to live with health, independence and dignity at home. The position is part‑time, 10 hours per week, allowing work‑life harmony.

Key Responsibilities

Conduct assessment visits with clients in accordance with CLA policies and state regulations.

Coordinate and manage care solution needs of clients by providing training for caregivers.

Evaluate the need for services and make referrals to other services as appropriate.

Develop a plan of care for the client, giving full consideration to the client's preferences for service arrangements and choice of caregivers. Secure written orders from the client’s physician, if applicable.

Provide administrative, coordination and direct service support for clients.

Coordinate and monitor clients served by Supported Living Providers.

Validate caregivers’ time and service through the electronic visit verification system (EVV).

Supervise, mentor and develop caregivers on RN’s caseload upon hire and throughout the provision of care.

Requirements and Qualifications

Associate’s Degree in Nursing

Current licensure as a Registered Nurse under s. 441.06, Wisconsin Statutes

Valid driver’s license, reliable transportation, and current auto insurance for travel throughout Dane County

Ability to lift up to 35 pounds

Work around environmental hazards such as odors, second‑hand smoke, animals, cleaning supplies and adverse living conditions

Satisfactory results of a WI Caregiver background check

Satisfactory results of a motor vehicle driver record check

Preferred Qualifications

Bachelor’s Degree in Nursing

One year of supervisory experience in personal care services, community‑based home care (non‑skilled nursing) or related program

Three years of nursing experience in personal care services or related program

Experience working with people with disabilities and elderly
