Independent Living Wellness Nurse (RN or LPN)

Join our Independent Living community as a Wellness Nurse, where your focus is helping residents maintain independence and thrivenot high-acuity bedside care.

Consistent resident population build real relationships

As a Wellness Nurse, you'll serve as a coordinator, educator, and advocate, supporting residents in staying healthy and independent.

Lead wellness programming and health education

Conduct assessments to support overall well-being

Connect residents with resources and services that help them remain in their apartments

Care Coordination

Coordinate weekly on-site physician clinic visits

Assist with lab orders and physician communication

Maintain accurate resident records, forms, and emergency contact information

Partner with the Healthcare Navigator to support residents and families

Guide transitions to higher levels of care when needed

Resident & Family Support

Respond to concerns and help navigate the aging journey

Active RN or LPN license (Illinois)

• 2+ years experience in geriatrics, community health, or case management preferred
• Medical, Dental, Vision Insurance
