Care Pavilion Nursing & Rehabilitation Center in Philadelphia, PA is seeking a Full-Time Physical Therapist (PT) to join our in-house rehabilitation team . We also have PRN opportunities available for therapists looking for additional flexibility.

What You'll Do
As a Physical Therapist, you will:

Evaluate residents and develop individualized plans of care

Provide skilled physical therapy services to SNF/LTC residents

Help residents improve strength, mobility, balance, safety, and functional independence

Establish measurable functional goals and monitor resident progress

Complete accurate and timely documentation

Collaborate with interdisciplinary team members to support positive resident outcomes

Provide resident, family, and caregiver education as appropriate

Qualifications

Graduate of an accredited Physical Therapy program

Current Pennsylvania Physical Therapist license or eligibility for licensure

Strong communication and clinical skills

Ability to work effectively as part of an interdisciplinary team

SNF/LTC experience is a plus

Experience with Net Health and PointClickCare (PCC) is a plus
