Job Purpose:
Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members. The LPN will report directly to the Director of Nursing.
Job Responsibilities:

Explaining services by performing community outreach. He or she will provide education to the clients, hospital staff, physicians, community stakeholders on Home Care Services and the admission process.
Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members.
Confer with the RN supervisor regarding needed changes in the Plan of Treatment/Care.
Provide nursing assessments and patient education.
Treat wounds, change wound dressings, giving injections, administer enema, and diabetic foot checks
Collect specimens from patients
Administers medications and set up medication dispensers.
Measures and records vital signs
