The typical US senior sees seven physicians across four practices plus insurers, pharmacies, family members, and facility staff. Fragmented data in incompatible systems leaves seniors and families managing chaos instead of focusing on living well.
Leland is building the first unified system of record for aging to help seniors live longer, healthier, and happier lives. With one complete record of a senior's health and life, AI can intelligently analyze the full picture to surface risks, gaps, and opportunities, while the senior's trusted human advocate guides the next best actions and removes friction so families can actually follow through. We have a multi-year funding runway and are launching pilots in 2026 with providers and already in active conversations with major US health systems. The company is led by a Stanford GSB founder who built and led Sword Health’s behavioral health division and co-founded Meomind.
You are Leland's first patient advocate, the person our first families call by name. You carry their care end to end: after-hospital follow-ups, referrals, medication reconciliation across pharmacies, insurance denials and appeals, being at or on appointments, and keeping the whole family updated. You work inside a physician-led practice with the living record behind you, so you always have the full picture and the routine work is drafted and tracked for you.

Capture what works, calls, scripts, escalation rules, and handoffs, so that every patient advocate who comes after you delivers the same quality you do.
3+ years of patient-facing work with older adults: navigation, care coordination, nursing, case management, or social work.
~ Warmth on the phone. Comfort working alongside AI tools that draft and track the routine work. Spanish is a big plus.

As Leland grows, this role can grow with it, potentially into leading care delivery and building the patient advocate team that comes after you. 90k-$135k base salary + equity.
