(RN) Registered Nurse - Home Health | Worcester, Massachusetts

Take the lead in delivering exceptional patient-centered care as a Registered Nurse specializing in Home Health. This full-time, 13-week contract role is based in a field environment, supporting patients in their homes within the Worcester area. The position offers a structured weekday schedule.

Schedule & Hours:

Monday through Friday, 8:00 AM 5:00 PM

Full-time, 13-week contract

Daily travel to patient homes within your assigned territory

Key Qualifications:

Active RN license in the state of practice, in good standing

At least one year of recent home health experience (field-based preferred)

Skilled in Home Care Home Base (HCHB) electronic documentation

Experienced in OASIS assessments and care planning

Reliable transportation, valid drivers license, and current auto insurance

Primary Responsibilities:

Complete thorough OASIS assessments and create individualized care plans

Deliver skilled nursing services, including wound care, IV therapy, medication management, and chronic disease monitoring

Evaluate patient progress, document any changes, and promptly share updates with care teams and physicians

Educate patients and families on conditions, medications, and care plans for better health outcomes

Maintain accurate, real-time documentation through HCHB

Work collaboratively with physical therapists, occupational therapists, social workers, and home health aides

Participate in regular case conferences, staff meetings, and ongoing education

Ensure compliance with agency and regulatory standards in both care and record-keeping

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
