RN Hospital Liaison for Hospice Care

Compassus

Compassus is seeking a Hospice Clinical Liaison in Ohio to model our values and promote the Compassus philosophy. You will assist referral sources in identifying patients whose needs align with hospice services and facilitate admissions and transitions from hospital or long-term care settings to home hospice care.
The role requires an RN license in the relevant state(s) and 2-3 years of related experience, with hospital/long-term care experience highly preferred.
