As a Licensed Practical Nurse (LPN) , you’ll play an essential role in enriching the lives of seniors by providing compassionate, high-quality nursing care while supporting resident safety, dignity, and well-being every single day.
As the Licensed Practical Nurse (LPN) , you provide hands-on nursing care in accordance with each resident’s individualized care plan. You support comprehensive health and psychosocial needs, monitor changes in condition, coordinate services, and collaborate closely with interdisciplinary team members to promote optimal resident outcomes in a community setting.
Provide nursing care to residents in accordance with established care plans, policies, and procedures.
Conduct ongoing assessments of residents’ physical and psychosocial status and monitor for changes in condition.
Administer medications and treatments as prescribed and document appropriately.
Coordinate and communicate resident care needs with physicians, families, pharmacies, and other care providers.
Support health promotion, disease prevention, and wellness initiatives within the community.
Assist with care planning, service plan updates, and resident evaluations as required.
Collaborate with caregivers, wellness team members, and leadership to ensure consistent, high-quality care delivery.
Respond to resident concerns, incidents, and emergencies using sound clinical judgment.
Support training and guidance for care staff as appropriate.
Ensure the community adheres to applicable federal, state, and local regulations related to resident care and services.
Participate in quality assurance initiatives, audits, and compliance activities as needed.
Current Licensed Practical Nurse (LPN) license, or a Professional Registered Nurse as per state regulations
Minimum of two (2) years of Licensed Nurse experience required.
Experience in assisted living, long-term care, or a skilled nursing environment preferred.
Demonstrated commitment to seniors and person-centered care.
Ability to work nights, weekends, and flexible schedules as required by the community.
Proficiency with electronic documentation systems and basic computer applications.
Ability to respond to medical emergencies.
Exposure to bloodborne pathogens and infectious disease.
Ability to lift or assist residents as required by care plans.

Certainpositions may be subject to additional requirements under federal programs,including CMS, Medicare, and Medicaid regulations, as well as state-specificlicensing or regulatory agencies. These measures support regulatory compliance and help maintain asafe, secure environment for residents, families, and team members.
Employmentin Florida is contingent upon completion of a Level 2 background screeningin accordance with Florida AHCA requirements. Applicants will be required tocomplete fingerprinting and background screening through the AHCAClearinghouse.
