Identifies need for and implements comprehensive nutrition assessments consistent with Ivinson Memorial Hospital policies; facilitates their active participation in establishing nutrition related goals and objectives. provides education and documents as appropriate in the medical record.
Determines patient centered outcomes that are clear, concise, observable and measurable, during the initial assessment and on follow up and documents consistently in the plan of care. Refers patients to appropriate resources in the community as needed.
Collaborates with other colleagues and health care professionals in expediting orders that lead to the best practice outcomes for the individual patient, this includes attendance at patient care conferences, rounds, etc. where care can be discussed and orders quickly obtained for required changes.
Performs nutritional assessments on all assigned dialysis patients per Departmental Standards of Care. Reviews lab results, dialysis adequacy, fluid weight status, and nutrition concerns with assigned patients on monthly basis, and educates patients regarding appropriate nutrition related changes when necessary.
Tracks Nutrition related QAPI data, participate in QAPI meetings and QAPI projects.
Participates in and coordinates departmental initiatives relative to patient care. May conduct education programs for patients, physicians, nurses, and other health care team members/departments, and/or the public.
Develops educational materials for use in developing the clinical nutrition skills of interns and students.
Bachelor's degree in Nutrition and Dietetics required.
Master's degree in Nutrition and Dietetics preferred.
One year of clinical nutrition experience preferred.
Must maintain Wyoming licensure for Registered Dietitians.
Must maintain registration with the Commission on Dietetic Registration.
Membership in the Wyoming Academy of Nutrition and Dietetics, Academy of Nutrition and Dietetics and Dietetic Practice Groups appropriate for specialty area preferred.
Certified Diabetes Care and Education Specialist credential preferred.
Excellent customer service skills, including but not limited to: a friendly personality, tact, patience, empathy and a helpful, professional attitude
Ability to effectively communicate with coworkers, hospital leadership, and the general public both verbally and in writing.
Decisive judgment and the ability to work effectively without immediate supervision.
Ability to learn new computer software programs.
Ability to recognize and protect confidential information.
Demonstrate ethical and legal accountability for the position.
