AM shifts, 5 × 8-hour shifts per week, totaling 40 hours.
Job Description
We are seeking an experienced Home Hospice Registered Nurse with expertise in providing care in a home health setting. The role involves managing patient care, conducting assessments, and ensuring compliance with hospice protocols.
Responsibilities
Conduct patient assessments and develop care plans.
Administer medications and provide necessary interventions.
Coordinate with interdisciplinary team members to ensure comprehensive care.
Document care activities and updates in the patient management system.
Maintain compliance with regulatory standards and protocols.
Required Experience / Certifications / Licensure
Current RN license.
Previous hospice experience is required.
Experience with HomeCare HomeBase (HCHB) is required; candidates without this experience may be considered at a reduced rate with training from management.
Valid driver's license and auto insurance are required.
Location
Coverage area includes Hemet, Menifee, and Murrieta, with potential for assignments within a 60-mile radius.
