Night Shift Counselor

Shift: 56 Hours - Sunday 9-5, Thursday 3pm - 10am (overnight) Friday 3p-8p Saturday 9am-9am (overnight)

Candidates must have related or direct experience working with the community we serve.

Promotes optimal health for individuals through routine, preventive, and emergency health care.

Responsible for assisting with all hands-on direct care which includes but not limited to: showering, toileting, shaving, brushing teeth, meal-time assistance

Provide education training, counseling, and physical assistance when necessary to achieve ISP goals in the following areas: functional academics, domestic skills health and personal hygiene, social and leisure skills, communication skills, community mobility, self-esteem, and money management.

Communicate with persons significant to the individual served (i.e. family, friends, guardians, funding sources, advocates, volunteers, employers) to facilitate consistency of programming and support.

Performs case management duties including, maintenance of individual confidential files, ensuring cases are reviewed, documentation of activities, and training. A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services.

Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
