Observe and report changes in resident condition to the Nurse Supervisor or physician/physician extender

Communicate with Supervisor,DON, NHA, care providers, and family/POA as needed

Use evidence-based practices to ensure safe, high-quality care delivery

Assist in managing personnel through performance evaluation, Care Chats, coaching, counseling, education, and corrective action as needed

Participate in resident transitions of care, care conferences, and care plan reviews

Collaborate with Cedarwood staff to ensure optimal resident outcomes

Comply with Infection Control practices

Ensure hand-held communication equipment is maintained for effective call bell management and team communication

Attend and complete all required in-services, training, and team meetingsAbide by the organization’s code of conduct

Registered Nurse (RN) in the Commonwealth of Pennsylvania

High school diploma or equivalent

Prior experience in geriatric nursing preferred

BLS Healthcare Provider certification

Basic computer skills

Commitment to person-centered care initiatives

Flexible schedule and willingness to work extra hours as needed
