Location: Vasona Creek Healthcare – Los Gatos/South Bay Area
Pay Rate: $62.00–$72.00+ per hour, DOE
Opportunities: Full-Time, Part-Time & PRN

We provide individual laptops to help therapists complete documentation efficiently, allowing you to spend more time focused on patient care and clinical outcomes.
Work alongside a multidisciplinary rehabilitation team of PTs, PTAs, OTs, and SLPs, supported by experienced clinicians with 10+ years of clinical experience. As a Physical Therapist, you will play an essential role in helping patients achieve their highest level of function, independence, and quality of life.
Perform comprehensive physical therapy evaluations and develop individualized, evidence-based treatment plans.
Provide skilled therapy services to short-term rehabilitation and long-term care residents.
Treat patients recovering from joint replacements, cardiac events, strokes, and other medical conditions.
Utilize specialized rehabilitation programs, including Acute Neuro Rehabilitation and Pain Management.
Collaborate with PTAs, OTs, SLPs, nursing, and other members of the interdisciplinary team.
Promote safe mobility, functional independence, and successful discharge planning.
Advocate for patients and support their individual rehabilitation goals.

Current California Physical Therapist (PT) license required.
Previous experience in skilled nursing, post-acute, or rehabilitation settings preferred.
Passion for rehabilitation and helping patients achieve meaningful functional outcomes.
Commitment to evidence-based, patient-centered care.
Ability to work collaboratively as part of a multidisciplinary rehabilitation team.
