As a home health nurse at Custom, you'll play a crucial role in empowering our patients to regain independence and improve their quality of life. Providing skilled care tailored to each patient's needs, facilitating their journey towards independence.
Conducting thorough initial assessments to develop personalized care plans in collaboration with physicians and the interdisciplinary team.
Educating patients and their families on ongoing care strategies and lifestyle improvements.
Coordinating and administering skilled treatments aimed at enhancing patient well-being and overall health.
Valid registered nursing license (required).

• 1+ years of registered nursing experience.
• Associate degree in nursing (required).
• Home health experience is a must.
