Care Management RN - Full-time

The Registered Nurse provides comprehensive nursing services, training, and support to individuals with developmental disabilities, brain injuries, substance abuse, or emotional challenges.
They conduct medical assessments, develop and supervise care plans, and ensure accurate documentation of medical records, medication administration, and treatment compliance.
They collaborate with physicians, promote health and wellness, and manage high-risk medical conditions, including private duty nursing.
The nurse offers medical skills training to clients, staff, and families, participates in health planning teams, and provides consultation on home care and medical equipment.
Additional duties include assisting with household activities, transportation, and participating in various team meetings.
Qualifications include a graduate of an accredited RN program, current state RN license, CPR/First Aid certification, and at least one year of experience with medically complex individuals.
Physical ability to perform heavy work and willingness to travel are required.
They must adhere to safety protocols involving potential exposure to blood and body fluids.
