RN - Evenings - Mental Health

Provides skilled nursing care to patients in a home health setting across assigned coverage areas. This role focuses on clinical excellence, patient education, and effective care coordination while meeting productivity expectations. The RN will complete OASIS assessments, manage complex clinical needs, and maintain accurate documentation using Homecare Homebase (HCHB).
Schedule: Monday through Friday, 8:00am to 4:30pm
Pay Rate: $53.00 per hour
One weekday evening per week from 4:30pm to 8:00am
One weekend per month from Friday 4:30pm to Monday 8:00am

~ Mileage Reimbursement: Yes, reimbursed between visits from first patient to last patient

Conduct comprehensive home health nursing visits, including Start of Care, Resumption of Care, Recertification, and Discharge visits
Complete accurate and timely OASIS assessments in compliance with home health regulations
Utilize Homecare Homebase (HCHB) for documentation and care planning
Provide skilled nursing services including wound care, wound vacuum management, ostomy care, Foley catheter care, blood draws, and PICC or IV care
Perform medication management, reconciliation, and patient education
Educate patients and caregivers on disease processes, medications, and care plans
Assess patient condition and communicate changes to the interdisciplinary care team
Maintain compliance with all agency policies, procedures, and regulatory standards
Active RN license
Associate's Degree in Nursing
Previous home health experience
Experience using Homecare Homebase (HCHB)
Clinical experience with wound care, wound vacs, ostomy care, blood draws, PICC or IV care, Foley catheter care, and medication management
RN license required
Professional certification beyond RN is not required
